Long before I was in the ward someone created a form for collecting data needed for the quarterly report. Clerks have just been making copies of copies ever since and its looking pretty bad. Last quarter someone wrote on the last copy so I'm making another collection form. Just something that a membership assistant clerk can write in the count and we keep track of class attendance.
First I was wondering if someone already had such a form already created that they would be willing to share.
Second the form has fields for each week and indicate that attendance, YM, YW, Primary, HT & VT are tracked for each moth and averaged for the report. However as this task is being turned over to me I've been reading and it looks like all that needed is the last month of the QTR. Is this correct? It would make the form much less complicated. I know the Bishop does like to keep track of SM attendance by the week even if it isn't needed.

Yes. Almost everything you provided for the quarterly report will be from the last month of the quarter.
