standard sustaining and release form

I am interested to know if there is standard sustaining/release form that you can use in the ward for that purpose as well as stakes. I am not interested in the one used for ward/stake conferences but one that is used almost weekly.

I have never seen a standard form. However, the correct wording is specified in the Handbook(s). When Elder Golden came presided over our stake conference last year, he asked us to double-check that we were using the correct wording, as specified in the Handbook(s).

The wording is in Handbook 2, Section 19.3. I would note that that wording is preceded by the statement "The officer may say" -- the word "may" implies that these exact words are not required, but it's certainly a great pattern to follow. For priesthood ordinations, the wording is in Handbook 1, Section 16.7.1.

where is the page that you read at conference the one that has all the leaders its our branch conference and i need that page

Well, I hope you found it at LCR under Reports > Officers Sustained.
