Proposed Calling(s)

I'm in a YSA Stake and so we have a number of releases and calling to be made every week. I recently was given the responsibility of tracking and updating these changes. Right now we have a paper form that is filled out for each proposed calling. I'm going to make it a fillable PDF form and here's my question.
How can I attach the form to a website so that it can be viewed in the browser filled out and then sent to me in an email?
I want the actual PDF filled out sent to me not the information that was typed into it.
Any ideas?
