I am looking for a form that grants our stake permission to use youth and leader's photos on a website (temporary TREK blog). I have looked at the release in the annual histories handbook but it does not quite fit. Does anyone know where I can find such a form - in digital format? Word? Pdf -fillable?

Was there any such forms supplied when the stake sought the permission needed to create the site as outlined in Handbook 2: 21.1.22?

If it's being organized under the "Members’ Use of the Internet in Church Callings", then I wouldn't think the stake would have anything to do with it.
