Does Bishop's Order for Commodities Form thread help? Or is this a different form?

If you know what form you are looking for, you can call the distribution center. If he wants it ordered online, he (the bishop) will have to do that himself.

Are you asking for a payment request form, similar to a reimbursement form? We use one in my ward, but there isn't a standard one. Each ward needs to create their own form to record the necessary information. Generally, our form isn't even something that gets handed out to people requesting assistance, I just attach it to bill that the bishop has agreed to pay so that all of the information can be recorded in one place.

Please refer to the two posts immediately preceding yours. If you are looking for the Storehouse form, it comes from LDS Distribution. If you are looking for some type of reimbursement form, you create those yourself.
