A clerk in my stake suggested adding a required “comment” field to pop up (similar to the window that comes up when you make a change to a financial expense or donation) on the Member Record Validation Report when you choose IGNORE.

This comment field would allow the clerk to justify the reason for pressing IGNORE, providing feedback to other clerks/Stake/HQ as to why the IGNORE option is being chosen. It could provide some history for understanding what has been done to solve the problem—and what did not work.

The previous entry makes a good comment. The only problem we have ever seen come up in the member validation report was when a member does not have both parents on their records. We have had members come and ask to have their parents removed from their IOS. Sometimes there is a good reason but stating it in a comment that is saved and sent out can document very confidential information, that has been taken care of by priesthood leaders.

In such cases the justification could simply be 'per request of member'. And there wouldn't be any need for further elaboration.
