There are no instructions in the Help menus of MLS about the "Child of Record (Consent Version)" form. When should we be using this form as opposed to to the regular "Child of Record" version? I assume there is a legal reason for it.

You are right about a legal requirement for this form. As far as I know, there are no areas in the United States and Canada that require use of this version of the form, but there are some other countries that do. If you are ever required to use this form, you will be notified by headquarters.
