Well, it is not exactly intuitive, but instead of a custom report:

On the Organizations panel,
Click on Relief Society.

In the Members section (the 3rd section down),
Click on the Print link near the top right side of the section.

In the List type to print drop down box at the top,
Select Birthday.
Fill out the Options.

Click the Print button (the one inside the box with the options).

Enjoy.

If you don't want the additional information (phone number, e-mail, and street address) provided by the method lajackson suggested but only want birthday and name or name and birthday then your custom report should have the following criteria.
attribute - Sex; compare operation - is; Value - female
attribute - Age: compare operation - is or is greater than; value -18.
Then on the "Format" tab make sure you have the "birthday' column selected. If you have not added any other columns then you should see "Preferred Name" and "Birthday" as the two columns in the "Column Order" list. That order will sort the sisters by name. If you want the sort order by birthday put the "Birthday" column first in the "Column Order" list.

This is a default RS directory. You don't have to do anything. Just go to Organizations > Relief Society. From there, I click on the "Print" hyperlink in the upper right corner of the box that lists all the sisters in the RS. In the popup window there is a drop-down menu. Click on the button and you'll see all the options (attendance roll, abbreviated directory, birthday list). Select Birthday list and print.

Is there a way to sort this by birthday month and day rather than by year/month/day. That is what you get with the regular sort, which gives you a list by age, not by month. This doesn't give the RS pres a convenient list by month and day?

Yes. If you follow the instructions in crislapi's comment immediately above yours, that is what you get.
