Who determines head of household?

Who determines who the head of household is? What are the factors that go into this decision?

I believe the principle factors are how it relates to home teaching and how the member wants to appear in the directory.
One popular policy among wards is that once a child leaves home (such as away for school), or gets married, they will be a separate household when they return.
