A little late for 2015, but when these were printed out, it showed the
usual boxes for the tithing status (full, part, non-tithe, and exempt ),
however it didn't print the boxes for who made the declaration
(Bishop or Member). We were all the way through tithing before
we noticed. The Bishop had to go back through all his attendance notes
to get that info for the final report. Also because we started Tithing Settlement
fairly early, some of the printouts were not real up to date. So, say a member
made no tithing contributions until say a couple of weeks in Dec. and then
didn't attend settlement, it would appear from the earlier report that they
were a non-tithe payer. It only becomes evident when filling out the final
status report that they were at least a part tithe payer. So those had to go
back to the Bishop for his declaration.

Contact your ward clerk. He can provide it.

The only sure way is to contact the ward clerk of each ward you donated to during the year.

In theory, all those donations should follow you to your current ward (and if you're in the US, the on-line donation system), but that's all dependent on all of your donations being correctly associated with your membership number.
