Report of Disciplinary Action - Current Version?

The Report of Disciplinary Action form I'm getting in MLS is dated 2/07, and the first line of instructions refers to Handbook I, Section 10 (2006).
Is this the current version version of the form?

It might be, but I highly recommend using the online tool at
lds.org/leader. Look under "Membership -> Confidential". It's much less confusing than the paper form and automatically routes itself for priesthood approval and submittal.

The one online would be the one you want to use. Like Mfmohlma said it is the easiest one to use as well.

I called CHQ and was told that the form in MLS is the latest and greatest, although it is slightly outdated since the issue of the new Handbook.
I have a very unusual situation, and thankfully, I’ve been given detailed instructions by Priesthood Leaders on what I need to do.
