Emergency Response Critical information Online Form

Forgive me if this has been discussed already, cannot find it: Can the church provide an Emergency Response Information Form containing critical info for Special Needs members , and the elderly of our ward, and even for the whole ward membership, we need a form that are filled and updated and submitted online so the ward/ stake can use them to expedite disaster/emergency responses.
The form could be done by families to avoid info repeats (family name, address, phones, emergency contacts) but give options / variables for special needs family members ( name , DOB and age of children/ members , with diabetes, with multiple disabilities, severe allergies, medication lists and dosages, meds instructions and special protocols, and other special needs) and last updates.
This is so needed so we can have a uniform way churchwide to access critical info for our members in distress, and the burden will shift from one person doing all the entering of the info and updating it to each able ward family member. Then the remaining members inot able to do this for themselves like the elderly and special needs would be drastically reduced for one person to enter on MLS and lds maps .

Not that I know of. I think most wards work through the HT/VT.

As someone who works quite a bit in emergency preparedness and disaster relief for our city, I sympathize with you in wanting to predefine where special needs people reside, however, there are specific privacy laws in place that need to be considered as well.

IMO, confidential information regarding people's disabilities and/or special needs should continue be kept at the local level among priesthood and Relief Society leaders.

There is a capability in LDS Maps to designate certain households for those who are emergency response leaders and those who have special needs.
