4) New Voice/fax Line orders do not include telephone instruments and are only for single line Plain Old Telephone Service (POTS)

Before I could possibly respond to any of these details, I would need to understand what the purpose of this form is. It seems to me that a community effort to create such a form is misplaced. Rather, whoever really needs the information should be specifying what is needed.

In my area, all such requests go through the FM group, and they need only a tiny fraction of the information you are suggesting. The proposed form suggests that the information will go to Xilec; if so, then shouldn't Xilec be creating the form?

You have provided a very interesting form that is not used by our FM group. And the provider you mention does not furnish service in our corner of the vineyard.

And that is our challenge. For many of our buildings, there is only one provider, and the Church does not have a contract with them.

Fortunately for us, our challenge was taken care of last March.

Xilec Telecommunications Inc, is a management company that is used by headquarters and is not a local telephone provider

Thanks for your input

Purpose is to have local telephone service installed and that the billing for service is made automatically and if your local FM group does not handle new telephone service request

The reason for the community effort is to address any needs that may have been overlooked and this is a joint effort with headquarters and their management company
