To request a new voice/fax line installation please complete as much as possible of the following:

1) Stake presidents in the United States and Canada Areas may authorize and fund, out of local unit budgets, unit's telephone service
2) Stakes will have their telephone bills paid directly by Church Administration and the expense will show on their monthly Church Unit Banking Services (CUBS) statement
3) Stakes outside of the United States and Canada should contact their Area Office
4) New Voice/fax Line orders do not include telephone instruments and are only for single line Plain Old Telephone Service (POTS)
5) Please notify your local LDS Facilities Management Group (FM) before submitting this request
6) Foyer or hallway telephone(s) should be restricted to local calls
