I'm looking for the language used from the pulpit when a new ward is created. I'm assuming that the person presenting the new ward describes the geographic boundaries of the ward or wards affected, and then asks for the new members of that ward to provide a sustaining vote. I can't seem to find it in the handbook and wondered if anyone knows where (or "if") it is available somehwere.

That's about the way we did it. I think the letter the stake president gets with the approval asks him to present the proposed division for a sustaining vote. Then the new bishop and his counselors are sustained.

When we divided two wards to make three recently, our stake president had a counselor describe the new boundaries. Then the president said that he was acting with permission from the First Presidency and Quorum of the Twelve and proposed that the One Ward and the Other Ward be divided to create the New Ward, as had previously been described. He asked for a sustaining vote, then if any were opposed. Then he asked for a vote of thanks as he announced the release of two bishops and their counselors. Then he presented three bishops and their counselors for a sustaining vote.

But as far as I know, there is no "official" language anywhere. Except that we did it in English. [grin]
