LDS Tools lists newly married couples under mother's last name

We have two families that recently moved in. Both couples in LDS Tools app (Android and IOS) appear under their mother's maiden name. When the indiviual is selected the app lists the correct first and last name. LDS.org always lists the correct name and household.

Example: Joseph Smith would be listed as "Mack, Joseph and Emma" in the app. Clicking on the name would put Mack in blue with the ward underneath. Household members would then be 'Smith, Joseph", "Smith, Emma" Clicking either name would place the correct name in the Blue.
Even stranger, when searching for "Smith" in Android or IOS app, Joseph would appear under the search results as "Joseph Smith".

In LCR on the Membership Information page select the household tab then select Edit Household Name
