LCR MP Ordination form fields blank when submitted from Ward to Stake

Hi, one of our ward clerks submitted a High Priest ordination for a member last Sunday. The person performing the ordination was from outside our Stake.

When I (Stake Clerk) received the notice of the ordination, I can see the proposed member and priesthood office. However, the fields for Ordination Date, and Who Performed are blank, so I'm unable to approve/record the ordination.

The information for the ordination date and who performed the ordination are not preprinted on the form. The bishop does not have that information. At the point the ordination recommendation is submitted to the stake, the ordination has not been completed yet.

When we converted these forms from MLS to LCR, we were instructed that performer information should not be preprinted, even in fairly obvious cases, such as a father baptizing or ordaining a child. You should print the form and have the high councilor or stake presidency member who oversees the ordination complete the information on the performer. That then gives you the information you will need to record the ordination.
