If this mysterious calendar is now a stake calendar, then it's no longer accurate to describe it as a ward calendar. You were most likely subscribed to it because it became a new stake calendar, and your Settings are configured to auto-subscribe to new calendars (that's the default setting).
gshosford wrote:1. How does this happen?
The most likely scenario is that some administrator changed the calendar from a ward calendar to a stake calendar. This is done by going into Settings > Calendars, clicking on a calendar, then changing the radio button labeled "This calendar is for" from the ward to the stake. The most likely suspect would be someone who is both a stake administrator and a ward administrator for the ward in question.

Nothing -- that is a built in feature that is useful. But once it happens, the calendar can be made a ward calendar once again. But you need a stake administrator who is also a member of that ward to do it.

Like I said, it's no longer that ward's calendar. So the best fix is to make it a ward calendar again; then you'll automatically be unsubscribed.
I'm not sure why your attempt to unsubscribe didn't stick. My only theory is that someone was playing with the calendar, perhaps disabling or removing it and then enabling or restoring it, which could trigger the auto-subscribe feature again.
For the short term, I'd recommend that you simply go into Settings > Subscriptions and try again to unsubscribe. That really should work.
