When I printed the Tithing Declaration form on 12 Nov 2017 to an HP LaserJet 1300, I found that the first character of the last name (at the left margin of the page) was partially cut-off. I recommend that you go to your new ward on Sunday and meet with the ward clerk or the membership clerk. They will collect the necessary information, and move the membership records for each member of your household into your new ward.

Sunday School attendance statistics are not included on the quarterly report! Unless the Sunday School attendance records are actually being used by the Sunday School Presidency, the teachers, or the bishopric, I see no reason to keep taking roll in Sunday School.
